The application for such license shall contain the following information:

    A.    Name and address of the applicant, age, date, and place of birth.

    B.    All prior convictions of felonies of the applicant, if any.

    C.    The address and name of business where the machine or device will be displayed and operated and the nature of the business conducted at the address under said name.

    D.    The name and address of the owner of the machine and if the machine is serviced and supplied by any person other than the applicant or owner of the machine, the name and address of such person shall be set out in the application.

    No license shall be issued to any applicant unless he is over eighteen (18) years of age and a citizen of the United States. (Ord. 1755 § 1 (part), 1999)
